CheckedUp Breakroom Sweepstakes Rules. No Purchase Necessary.
Eligibility: CheckedUp Breakroom Sweepstakes is open exclusively to verified partners who have received this letter. Entries must be unique and cannot be duplicated. The number of practices that this promotion is sent to will be determined by CheckedUp.
Entry Period: The Sweepstakes begins on April 1, 2024, and ends on October 1, 2024.
How to Enter: Participating practices must submit a photo of the CheckedUp HCP Poster displayed in the breakroom or back office to [email protected]. Entries must include practice name, address, and phone number. Entries must be submitted by the address where the posters were shipped and cannot be duplicated. Each valid entry qualifies for a $25 Coffee Gift Card. Limit one gift card per practice. Each completed photo submission (one entry per practice) will be automatically entered into the CheckedUp Breakroom Sweepstakes.
Sweepstakes Prizes: Participants will receive a $25 Coffee Gift Card for submitting proof of the poster display. Additionally, participants will be entered into the CheckedUp Breakroom Sweepstakes. Total Approximate Retail Value (ARV): $2,500.
Winner Selection: The sweepstakes winner will be selected randomly via 3rd party on or around November 1, 2024.
Notification of Winners: Winners will be notified no later than December 1, 2024 by phone and email. One winner will have 60 days to claim their prize. Failure to respond within the specified timeframe may result in disqualification.
Disqualification: No duplicate entries allowed, submissions must include a clear photo of the CheckedUp HCP Poster in the designated location, entries must originate from the address to which posters were shipped, no manipulated photos allowed, entries submitted after the deadline will not be accepted, and any violation of sweepstakes terms may lead to disqualification.
Limitation: Only US practices who were sent posters are eligible to enter the promotion and sweepstakes.
Privacy Policy: The collection and use of personal information will be governed by CheckedUp’s Privacy Policy.
Taxes and Fees: Winners are solely responsible for any taxes or fees associated with the prizes they receive.
Publicity Release: By entering the CheckedUp Breakroom Sweepstakes, participants grant CheckedUp the right to use their practice name and the submitted materials for promotional purposes without additional compensation.
Limitation of Liability: CheckedUp is not responsible for any liability arising from technical malfunctions, entries not received, or any other issues that may affect the fairness or integrity of the Sweepstakes.
Modification or Termination: CheckedUp reserves the right to modify or terminate the CheckedUp Refresh Sweepstakes at any time, for any reason.
Compliance with Laws: Participants must comply with all applicable federal, state, and local laws and regulations.
